Resource Spotlight: Mywell Health

If you’re looking for a simple way to take control of your health and lifestyle planning — especially when facing a health change or transition — we wanted to highlight a great resource: Mywell Health.

Mywell Health offers accessible, age-appropriate planning tools and activities at no cost, to help you talk, record, and share your health and lifestyle information with family, friends or your health‐care team. Whether you’re in the “adults” category (ages 30–64), are caring for a senior (65+) or have health ⚠️ changes you’re navigating, these tools are designed to empower you. (And a youth version for ages 14–29 is coming in Fall 2025.)

One of the things we appreciate about this tool is its focus on helping you be prepared for meaningful conversations: questions like “What matters most to me?”, “What lifestyle choices do I want to prioritise?”, “How do I want my care to proceed if things change?” — are surfaced in the planning activities. Once you complete the printable/fillable tools you can digitally or physically share them with people you trust. It’s a way to take an active role rather than passively receiving care.

Importantly, Mywell Health does not store your personal health information or completed tools — so you retain full control over your data. That said, it is not a substitute for medical advice or an emergency care tool. If you have urgent concerns, you should always speak with your physician or health team.

This kind of self-directed planning tool can complement the support your provider or care-team gives you. It may be especially helpful if you’re wanting to reflect on changes in your health, prepare for an appointment, review your lifestyle goals, or ensure your loved ones understand what matters to you.

If you’d like to explore: go to https://mywellhealth.info and click “Get Started” under your age-group category. You’ll find the Resource Kit (glossary, educational links, videos) and the planner-activities.

Tip for getting the most out of it:

  • Set aside 30 minutes in a quiet space (with your device or printouts) to fill in one module.
  • Consider doing it with a care-partner, family member or friend — the “talk” element is built in.
  • Save the filled tool in a folder you can revisit, and bring it to your next appointment or share with your provider ahead of time.
  • If you’re a clinician or part of a care team, you might consider printing one of the modules and leaving it in your waiting room as a prompt for patients.

We hope this resource helps you feel more empowered, prepared and clear about your health-and-lifestyle decisions. If you use it and find it valuable, we’d love to hear how it worked out for you — drop us a note.
